For Discharge Planners

We are eager to work and partner with discharge planners and other healthcare professionals for the benefit of their patients and referrals. By way of a reminder, here are some of the features, services and differentiating factors that help make Garden Care Center special:

• Our short term rehab is available 7 days a week
• We are also happy to discuss our Outpatient Rehab Program and Long Term Care services with your patients
• Medicare, Medicaid and most HMOs are accepted
• Our Admissions Office is staffed Monday through Friday from 9:00 AM to 5:00 PM
• The Admissions Coordinator is on call weekends to accommodate weekend admissions
• An RN Case Manager handles all HMO requirements
• Our outreach staff members are always in the field and available to see any patient and conduct evaluations for appropriate placement
• The latest medical technologies and special equipment (wound vac, bipap, cpm, etc.) are available as needed
• Fast response to PRIs is assured – we pride ourselves in working closely with discharge planners – making your job easier, makes our job easier
